Job Role / Responsibilities:
You are required to:

  • act as a point ofcontact person for patients under the umbrella of GI & Surgery
  • coordinate with teammembers and healthcare professionals of other disciplines, for responding oncare needs of Gi & Surgerypatients
  • promote efficientdelivery of patient care throughout the healthcare system
  • actively participatesin quality improvement initiatives, utilizing information from relevant sources
  • apply principles ofteaching/learning to design, provide and evaluate education delivered topatient and families
  • exhibit skills foreffective communications and team building
  • facilitates Nursingto implements and evidence-based nursing interventions for common illnesses in GI& Surgery
  • ensure patientdischarge plan is initiated timely and patient is planned and executedefficiently
  • orients and educatespatients and their families by meeting them; explaining the role of the patientcare coordinator; initiating the care plan; providing educational informationin conjunction with direct care providers related to treatments, procedures,medications, and continuing care requirements
  • developsinterdisciplinary care plan and other case management tools by participating inmeetings; coordinating information and care requirements with other careproviders; resolving issues that could affect smooth care progression;fostering peer support; providing education to others regarding the casemanagement process
  • maintaining ongoingcommunication with utilization review staff regarding variances from the care planor transfer/discharge plan
  • complies withhospital and legal requirements by fostering nursing practices that adhere tothe hospital’s goals, and standards of care
  • respects patients byrecognizing their rights; maintaining confidentiality.maintains quality serviceby establishing and enforcing organization standards
  • facilitates in maintainingpatient care database by entering new information as it becomes available;verifying findings and reports; backing up data
  • reviewing & maintenanceof patient care areas

Eligibility Criteria / Requirements:

You should have:

  • Bachelors’s or Master’s degree preferably MBA
  • minimum of three to five years of experience in marketing preferably in health care environment
  • sound persuasive skills
  • willingness to travel
  • willing to work in shifts
  • good communication skills (written/verbal)
  • sound knowledge of computer software including data maintenance, data analysis.
